Cacao and Reiki Healing Circle Event Liability Waiver

I acknowledge that this event will take place in person and will include ceremonial grade cacao (dairy & gluten free but contains coconut), reiki healing, and meditation. I hereby assume all of the risks of participating in this event.

I certify that I am healthy and do not have any preexisting conditions that would prevent me from participating in this event and have not been advised otherwise by a qualified medical person.

I acknowledge that this Accident Waiver and Release of Liability (AWRL) form will be used by REAL HEALTH WITH RACH, and the sponsors of the event in which I may participate and it will govern my actions and responsibilities at the event.

In consideration of my decision to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

(A) Waive, release, and discharge from any and all liability for my death, disability personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me or my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Rachel Hoeppner, Real Health with Rach, and Monique Gomez, and their directors, officers, employees, volunteers, representatives and agents, the event sponsors and event volunteers,

(B) indemnify and hold harmless all entities or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a result of my actions during this event.

I hereby consent to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during this event.

I understand that at this event or related activities I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers, and/or assigns.

This AWRL shall be construed broadly to provide a release and waiver to the maximum extent permissible under the applicable law.

Refund Policy: All ticket sales are final, unless the event is cancelled or postponed by the event organizers. By purchasing a ticket, you agree to abide by this no refunds policy.

This agreement shall be construed according to the laws of the State of Arizona. In the event that any provision of this Agreement is deemed unenforceable, the remaining portions of the Agreement shall be severed and remain in full force.

By purchasing this Offer, and checking the box that you "Agree to the Terms and Conditions" you are confirming that the terms of this Agreement are acceptable. You, the Client, acknowledges that: (1) he/she has received a copy of this AWRL; (2) he/she has had an opportunity to discuss the contents with the Coach (Rachel Hoeppner and/or Monique Gomez) and, if desired, to have it reviewed by an attorney; and (3) the client understands, accepts and agrees to abide by the terms hereof.
